Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.

Theoretically, chokeberry might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.

Here's the gist for Selinexor (Xpovio). Your body clears this cancer drug partly through a liver enzyme called CYP3A4. Lab studies suggest chokeberry can slow that enzyme down. In theory, if chokeberry blocks CYP3A4, Selinexor could clear more slowly and build up, which might mean stronger effects and more side effects like nausea, vomiting, poor appetite, fatigue, and low blood counts.

I want to be honest about how thin the evidence is. This is mostly test-tube data plus one case report involving a different drug, so it's far from proven. Still, with a potent chemo drug, even small shifts matter. Eating an occasional berry or bit of jam is unlikely to be a problem, but please check with your oncology team before using concentrated chokeberry supplements or juice.

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
