Clove + Fentanyl
Fentanyl brand names: Sublimaze, Duragesic, Fentora, Lazanda, Abstral, and others.
Fentanyl falls into 2 drug categories that Clove can interact with; the most serious is rated moderate. Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP2D6.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Clove interacts with whole categories of medication. Fentanyl falls into 2 of those categories, so it can interact with Clove in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Fentanyl
Fentanyl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Clove interacts with.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP2D6 in a dose-dependent manner. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with clove and fentanyl. Clove's main compound, eugenol, has been shown in lab dish studies to slow down an enzyme called CYP2D6. The worry is that if that enzyme runs slower, a drug it clears could build up in your body. Fentanyl is a strong opioid, and even small increases in its level matter because too much can cause heavy sedation, dangerously slow breathing, and overdose.
That said, this is theoretical. It has only been seen in the lab, never confirmed in people, and fentanyl is actually broken down mostly by a different enzyme (CYP3A4), so CYP2D6's role is limited. Cooking with clove almost certainly won't matter. Concentrated clove oil or supplements are a different story, so check with your pharmacist or doctor before using them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Fentanyl also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP3A4 in a dose-dependent manner. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection worth knowing: your body relies on a liver enzyme called CYP3A4 to break down fentanyl. Lab studies suggest that eugenol, the main compound in clove, may slow that same enzyme down. If that happened in real life, fentanyl could clear more slowly and build up in your system, leading to stronger opioid effects like heavy drowsiness, confusion, very slow breathing, or dangerously low blood pressure. With a powerful drug like fentanyl, even small shifts deserve respect.
That said, this is purely theoretical, based on test-tube research and never shown in people. A sprinkle of clove in cooking is very unlikely to matter. Concentrated clove oil or supplements are the bigger question. Please check with your pharmacist or doctor before combining them.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

This page covers Clove only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Fentanyl page lists 226 supplements it has a documented interaction with, worst first, each one explained.
The most serious there right now: 1,4-butanediol (major), Activated Charcoal (major), Beer (major), Gamma-hydroxybutyrate (ghb) (major) and Grapefruit (major).
By severity: 10 major · 176 moderate · 40 minor.
From our pharmacist’s overview on the Fentanyl page
“There is very little room for error with fentanyl (Duragesic, Sublimaze, Actiq, and other forms), a powerful opioid where a modest rise in blood level or an extra layer of sedation can become dangerous, so this interaction list deserves real respect. Most of the risk falls into two patterns.”

About Clove
What is Clove?
- Toothache and oral pain
- Digestive upset
- Antimicrobial and antifungal
- Cough and respiratory complaints
- Flavoring in food and drinks
Clove is a common cooking spice that is also used in traditional medicine, especially as a topical numbing agent for tooth pain thanks to its main compound, eugenol. Food amounts are generally safe, but concentrated clove oil and supplements can be irritating or toxic if overused, and human evidence for most health claims is limited. Talk to your pharmacist or doctor before using clove medicinally, especially in children or during pregnancy.
