Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Clove interacts with.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP2D6 in a dose-dependent manner. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this one. Your body relies on a liver enzyme called CYP2D6 to clear nortriptyline out of your system. Lab studies suggest that eugenol, the main compound in clove, can slow that enzyme down. If that happened in people, nortriptyline could build up a bit higher than expected, which might mean more of its usual side effects like dry mouth, constipation, drowsiness, dizziness when standing, or a faster heartbeat.
I want to be honest about the evidence, though. This has only been seen in test-tube studies, not in actual patients, so it's more theoretical than proven. Everyday cooking with clove almost certainly won't matter. If you're thinking about concentrated clove oil or supplements, just check with your pharmacist or doctor first.

Here's the idea behind this pairing. Your liver uses an enzyme called CYP1A2 to help clear nortriptyline. Lab studies suggest that eugenol, the main compound in clove, can slow that enzyme down. If that happened in your body, less of the drug would be broken down and levels could creep up, which might mean stronger side effects like dry mouth, constipation, drowsiness, dizziness, blurry vision, or a faster heartbeat. Nortriptyline isn't a prodrug, so more of it simply means more effect.
The reassuring part: this is only seen in test-tube research, not in real people, and a sprinkle of clove in your cooking is very unlikely to matter. Concentrated clove oil or supplements are where I'd be more careful. Check with your pharmacist before using those.
