Coffee Charcoal and Alprazolam

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ORAL DRUGS

Why this applies to Alprazolam

Alprazolam falls under ORAL DRUGS — the group Coffee Charcoal interacts with.

Coffee charcoal can reduce the absorption of orally administered drugs and should be separated from oral drug administration by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: coffee charcoal works a lot like other charcoals, with a porous surface that can grab onto things in your gut. If you swallow your alprazolam (Xanax) at the same time as coffee charcoal, some of the medicine may stick to the charcoal instead of getting absorbed into your body. That could mean your dose works less well than usual, so your anxiety or panic symptoms might not be controlled as smoothly as you'd expect.

This is a theoretical concern based on how charcoal behaves, not a documented disaster, so don't panic. The easy fix is timing. Keeping them at least two hours apart usually avoids the problem. If you take coffee charcoal often, just check in with your pharmacist or doctor first.

From our pharmacist’s overview on the Alprazolam page

“Sedation stacking is the core danger with alprazolam (Xanax), a benzodiazepine for anxiety and panic, and this list deserves genuine care rather than a quick skim. Dozens of entries, alcohol chief among them, deepen its drowsiness and can slow breathing, while an even larger group changes how the body clears the drug through the CYP3A4…”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.
