Coffee Charcoal and Anastrozole

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ORAL DRUGS

Why this applies to Anastrozole

Anastrozole falls under ORAL DRUGS — the group Coffee Charcoal interacts with.

Coffee charcoal can reduce the absorption of orally administered drugs and should be separated from oral drug administration by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: coffee charcoal acts like a sponge in your gut. Because of its porous, charcoal-like surface, it can grab onto things you swallow, including your anastrozole (Arimidex), before your body has a chance to absorb the medicine. If less of your daily dose gets into your bloodstream, the drug may not lower your estrogen as well as it should, which matters a lot when you're treating breast cancer.

The good news is this is a timing issue, not a reason to panic. Keep in mind this concern is theoretical, based on how charcoal behaves rather than studies in people. To stay safe, separate the two by at least two hours, and check with your pharmacist or doctor before adding coffee charcoal.

By severity: 1 major · 12 moderate · 9 minor.

From our pharmacist’s overview on the Anastrozole page

“Activated charcoal carries the one major rating on the anastrozole (Arimidex) list, because it can soak up the drug in the gut and cut how much your body absorbs. For a medicine that treats hormone-sensitive breast cancer by keeping estrogen suppressed, anything that lowers its level deserves real concern, and I would act on this list…”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.

About Anastrozole

What Is Anastrozole Used For?

Brand name: Arimidex

Anastrozole (also sold as Arimidex) is used in postmenopausal women to treat hormone receptor-positive breast cancer at several stages. This includes early breast cancer after surgery (adjuvant therapy), locally advanced or metastatic breast cancer as a first treatment, and advanced breast cancer that has continued to grow after tamoxifen therapy.

For the second-line use in advanced breast cancer, women whose cancer did not respond to estrogen receptor (ER) testing or who did not respond to tamoxifen rarely benefited from anastrozole.
