Here's the simple version: coffee charcoal works like other charcoals by acting like a sponge in your gut. If you swallow it too close to your Tussend (which contains chlorpheniramine, hydrocodone, and pseudoephedrine), the charcoal can grab onto those medicines before your body absorbs them. That means less of each ingredient gets into your bloodstream.

In real terms, your cough and congestion relief might not work as well. You could get less drowsiness from the chlorpheniramine, weaker cough suppression from the hydrocodone, and less decongestant effect. This is based on how charcoal behaves, not strong studies, so don't panic. To stay safe, just space them out by at least two hours, and check with your pharmacist if you're unsure.
