Here's the gist with coffee charcoal and cilostazol (Pletal): that charred, porous charcoal can bind to medications sitting in your stomach and intestines. If they're in there at the same time, less of your cilostazol may actually get absorbed into your body. When that happens, the drug may not work as well, meaning your leg pain from walking (intermittent claudication) might not improve the way it should.

The good news is this is a theoretical concern based on how charcoal behaves, not strong human studies. A simple fix usually helps: separating them by at least two hours so the charcoal isn't soaking up your dose. Before doing this, please check with your pharmacist or doctor so you keep getting the full benefit of your cilostazol.
