Here's the practical concern with Xtandi (enzalutamide) and coffee charcoal. Like other charcoal products, coffee charcoal has a porous surface that can grab onto medications sitting in your gut. If you swallow them too close together, some of your enzalutamide may bind to the charcoal and pass through without being absorbed. That means a smaller dose actually reaches your bloodstream, which could make your prostate cancer treatment less effective over time.

To be clear, this is a theoretical concern based on how charcoal behaves, not strong study data. Still, with a cancer drug you want full absorption, so spacing them by at least two hours is sensible. Please run this by your oncology pharmacist or doctor before adding coffee charcoal.

About Enzalutamide

What Is Enzalutamide Used For?

Brand name: Xtandi

Xtandi (enzalutamide) is used to treat prostate cancer in three specific situations: castration-resistant prostate cancer (CRPC), metastatic castration-sensitive prostate cancer (mCSPC), and non-metastatic castration-sensitive prostate cancer with biochemical recurrence at high risk for metastasis (high-risk BCR).

Castration-resistant means the cancer has continued to grow despite low testosterone levels. Castration-sensitive means the cancer still responds to testosterone-lowering therapy. In most of these cases, Xtandi is used alongside a GnRH analog (a hormone-lowering injection) or after surgical removal of the testicles — except in the high-risk BCR setting, where it may be used with or without a GnRH analog.
