Penicillin V falls under ORAL DRUGS — the group Coffee Charcoal interacts with.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: coffee charcoal acts like other charcoals, meaning it has a porous surface that can grab onto things sitting in your gut. If you swallow your Penicillin V at the same time, some of that antibiotic may stick to the charcoal instead of being absorbed into your bloodstream. That could leave you with less penicillin actually fighting your infection, and an antibiotic that doesn't fully do its job is a real concern.
This is based on how charcoal behaves rather than hard studies with this exact drug, so it's more of a sensible precaution than a proven problem. The easy fix is timing. Keep your penicillin and the coffee charcoal at least two hours apart, and check with your pharmacist if you're unsure.
