Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: coffee charcoal acts a bit like other charcoals in your gut. Its porous surface can grab onto things, including your pentoxifylline tablet, before your body has a chance to absorb it. If they're sitting in your stomach at the same time, you may end up taking in less of the medicine than intended.

For pentoxifylline, that could mean it doesn't work as well, so the leg aching, cramping, and tiredness it's meant to ease might not improve as much. This concern is mostly theoretical, based on how charcoal behaves rather than firm studies. An easy fix is spacing them apart by at least two hours. Please run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Pentoxifylline page

“Pentoxifylline (Trental) improves blood flow in people with poor leg circulation partly by making platelets less sticky, so treat this list as one to read carefully rather than skim. Of the 218 supplements on file, 202 can also slow clotting, which means adding them stacks bleeding risk on bleeding risk.”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.

About Pentoxifylline

What Is Pentoxifylline Used For?

Brand name: Trental

Pentoxifylline Extended-Release Tablets are used to treat intermittent claudication — leg pain, cramping, or fatigue that happens during walking because narrowed arteries aren't delivering enough blood to the muscles. The underlying cause is chronic occlusive arterial disease, a condition where the arteries in the limbs become blocked or narrowed over time.

Pentoxifylline can help improve walking distance and reduce symptoms, but it is not a substitute for procedures that physically open or bypass blocked arteries when those treatments are needed.
