Here's the simple version: coffee charcoal works like a sponge in your gut. That porous, charred surface can grab onto medicines you swallow before your body has a chance to absorb them. With pyrazinamide, an important part of your TB treatment, the worry is that less of the drug actually gets into your bloodstream.
That matters because TB medicines need steady, full levels to keep killing the bacteria. If absorption drops, you may not get the full benefit, which can let the infection linger or come back. To be safe, it's smart to space coffee charcoal at least two hours away from your pyrazinamide dose. This concern is based on how charcoal behaves, not hard studies, but with TB drugs I'd still play it safe. Please run it by your pharmacist or doctor first.

What Is Pyrazinamide Used For?

Pyrazinamide tablets are used to treat active tuberculosis (TB) in adults and children. It is always used in combination with other anti-TB medicines — never alone. The standard CDC-recommended approach uses pyrazinamide as part of a 6-month regimen, typically for the first 2 months.
It is also used when initial TB treatment with other drugs has failed. People with drug-resistant TB or with HIV infection may need a treatment plan specifically tailored to their situation, which can include pyrazinamide as a key part of the regimen.
