ORAL DRUGS
Why this applies to Vismodegib
Vismodegib falls under ORAL DRUGS — the group Coffee Charcoal interacts with.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: coffee charcoal works a bit like activated charcoal, meaning its porous surface can grab onto things in your gut. If you take it too close to your vismodegib (Erivedge) dose, it may soak up some of the medicine before your body absorbs it. That could leave less drug in your system to do its job fighting the basal cell carcinoma.
With a cancer therapy like this, you really want the full dose working, so getting less than intended isn't something to shrug off. The good news is this concern is theoretical, based on how charcoal behaves rather than firm studies. A practical fix is spacing them out by at least two hours. Please run this by your pharmacist or oncology team first.

This page covers Coffee Charcoal only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Vismodegib page lists 57 supplements it has a documented interaction with, worst first, each one explained.
The most serious there right now: Activated Charcoal (major), St. John's Wort (major), Bitter Melon (moderate), Black Pepper (moderate) and Bulbine Natalensis (moderate).
By severity: 2 major · 38 moderate · 17 minor.
From our pharmacist’s overview on the Vismodegib page
“St. John's wort can quietly undercut vismodegib (Erivedge), the pill for advanced basal cell carcinoma, by speeding up the gut pump that clears the drug from the body, and a cancer treatment that silently loses potency is the worst kind of interaction.”

About Coffee Charcoal
What is Coffee Charcoal?
- Diarrhea
- Inflammatory bowel conditions
- Irritable bowel syndrome (IBS)
- Mild digestive upset
- Wound and skin care (topical, traditional)
Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.

Brand name: Erivedge
Erivedge (vismodegib) is used to treat adults with basal cell carcinoma — the most common type of skin cancer — in two specific situations: when the cancer has spread to other parts of the body (metastatic basal cell carcinoma), or when it is locally advanced and has returned after surgery, or when surgery and radiation are not options.
It is not approved for use in children.
