Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Coltsfoot interacts with.

Concomitant use might increase the risk of bleeding due to decreased platelet aggregation. Coltsfoot has been reported to inhibit platelet aggregation; avoid concomitant use. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing about alteplase (Activase): it's a powerful clot-busting drug given in the hospital during emergencies like a heart attack, stroke, or a serious blood clot in the lungs. It works fast and strong to dissolve clots, and its main risk is bleeding. Coltsfoot has been reported in lab work to slow down platelet aggregation, meaning it may make it a little harder for your blood to form clots on its own.

Put those together and you could, in theory, raise the bleeding risk a bit more. I'll be honest, this concern is theoretical and based on how the two work, not on real patient studies. Realistically, alteplase is given in a controlled hospital setting where you wouldn't be taking herbs alongside it. Still, please tell your medical team about any supplement use, and don't start or stop anything on your own.

Coltsfoot is a traditional herb long used to soothe coughs and clear mucus, but solid human evidence for these uses is lacking. More importantly, the raw plant can contain liver-toxic pyrrolizidine alkaloids, so it carries real safety concerns and is best avoided unless you use a product certified to be free of these compounds.
