Here's the short version: Bosentan helps relax the blood vessels in your lungs to ease pulmonary arterial hypertension. The concern with coltsfoot is mostly theoretical and rests on weak, anecdotal reports. The idea is that large amounts of coltsfoot might work against the goal of your blood pressure and heart medicines, potentially blunting how well bosentan does its job.

Honestly, my bigger worry with coltsfoot isn't this interaction at all. The raw plant can contain compounds that are toxic to the liver, and bosentan already carries its own risk of liver strain. Stacking those together isn't wise. Food-level flavors aren't the issue here, concentrated supplement doses are. Please check with your pharmacist or doctor before using any coltsfoot product.

Coltsfoot is a traditional herb long used to soothe coughs and clear mucus, but solid human evidence for these uses is lacking. More importantly, the raw plant can contain liver-toxic pyrrolizidine alkaloids, so it carries real safety concerns and is best avoided unless you use a product certified to be free of these compounds.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
