P-GLYCOPROTEIN SUBSTRATES

Apremilast falls under P-GLYCOPROTEIN SUBSTRATES — the group Eleuthero interacts with.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist of this pairing. Apremilast (Otezla) is partly moved around your body by a transporter called P-glycoprotein. In lab (test tube) studies, eleuthero appears to slow that transporter down a bit. In theory, that could let a little more apremilast linger in your system, which might nudge up its usual side effects like diarrhea, nausea, headache, or feeling low in mood.
The important part: this is only theoretical. It has never actually been reported in people, and the evidence comes from cell studies, not real patients. Apremilast is not a prodrug, so it still works the way it normally does. I wouldn't lose sleep over this, but if you want to add eleuthero, give your pharmacist or doctor a quick heads-up first, especially if tummy upset bothers you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Apremilast also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for Otezla (apremilast): your body clears it mainly through a liver enzyme called CYP3A4. Eleuthero has been looked at as something that might slow that enzyme down. If it did, apremilast could build up a bit and you'd be more likely to feel its usual side effects, like nausea, diarrhea, headache, or low mood.
But honestly, I wouldn't lose sleep over this one. The concern comes from lab and animal tests, while an actual study in healthy people taking eleuthero twice daily for two weeks showed no meaningful effect on CYP3A4. The interaction is rated minor and unlikely. Still, if you plan to use eleuthero regularly, give your pharmacist or doctor a quick heads-up so they can keep an eye on how you're doing.

What Is Apremilast Used For?
Brand name: Otezla
Apremilast treats several inflammatory conditions. In adults, it is used for active psoriatic arthritis, plaque psoriasis in people who are candidates for phototherapy or systemic therapy, and oral ulcers associated with Behçet's Disease.
Children 6 and older can use it for active psoriatic arthritis and moderate to severe plaque psoriasis. Otezla is for children weighing at least 20 kg. Otezla XR is for children weighing at least 50 kg. Neither product is established for Behçet's Disease in children.
