Buproprion, Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP2D6.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP2D6. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP2D6 drug metabolism.

Here's the situation with Auvelity (bupropion plus dextromethorphan) and eleuthero. The dextromethorphan part is cleared by an enzyme called CYP2D6, and the bupropion in this combo is actually there to slow that enzyme down so the dextromethorphan sticks around and works. In lab and animal studies, eleuthero looked like it might also block CYP2D6, which in theory could nudge dextromethorphan levels up a bit more, potentially adding to side effects like dizziness, drowsiness, or nausea.

The reassuring news: when actual people took eleuthero for two weeks, it did not meaningfully affect CYP2D6. So this is a minor, mostly theoretical concern. I still suggest mentioning eleuthero to your pharmacist or doctor so they can keep an eye on how you feel.

Auvelity combines bupropion and dextromethorphan, and one piece of this combo (dextromethorphan) leans on the CYP3A4 enzyme to be cleared from your body. On paper, eleuthero might slow that enzyme, which could let dextromethorphan build up a bit and bring on things like dizziness, drowsiness, or a foggy, lightheaded feeling. The bupropion side can already nudge up blood pressure and lower seizure threshold, so you don't want extra unknowns piled on.

Here's the reassuring part: this is mostly theory from lab and animal studies. When healthy people actually took eleuthero for two weeks, it did not meaningfully block CYP3A4. The real-world risk looks low. Still, it's worth a quick word with your pharmacist or doctor before combining them.
