Erdafitinib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP2C9.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP2C9. This effect has not been reported in humans.

Here's the gist for erdafitinib (Balversa). Your body partly clears this cancer drug using a liver enzyme called CYP2C9. In lab dishes and animal studies, eleuthero appeared to slow that enzyme down. If the same thing happened in people, erdafitinib could build up a bit higher than expected, which might mean more of its known side effects, things like high phosphate levels, mouth sores, dry or sore eyes and vision changes, dry mouth, nail problems, and diarrhea.

I want to be honest about the strength of this, though. It comes only from test tube and animal research, and it has never actually been shown in humans, so treat it as a theoretical concern rather than a proven problem. Since erdafitinib is a serious cancer therapy, please don't add or stop eleuthero on your own. Run it by your oncologist or pharmacist first.

Erdafitinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the gist for Balversa (erdafitinib). Your body uses a liver enzyme called CYP3A4 to help clear this cancer medicine. In the test tube and in animals, eleuthero looked like it might slow that enzyme down a little, which in theory could let erdafitinib build up and lead to more of its usual side effects (things like dry mouth and eyes, mouth sores, nail or skin changes, and vision problems).

The reassuring part: when researchers gave healthy people eleuthero for two weeks, it did not actually change CYP3A4 activity. So this is mostly a theoretical concern, and erdafitinib is not a prodrug, so the direction would be slightly higher levels, not weaker effect. Still, because this is a serious cancer drug, please run any supplement past your oncologist or pharmacist before starting it.

About Erdafitinib

What Is Erdafitinib Used For?

Brand name: Balversa

Balversa (erdafitinib) is used to treat adults with locally advanced or metastatic urothelial carcinoma (UC) — cancer of the bladder or urinary tract — that has spread and has not responded to, or has gotten worse after, at least one prior round of systemic treatment. Patients must have a specific type of FGFR3 genetic alteration in their tumor, confirmed by an FDA-approved test before starting.

Balversa is not recommended for patients who are eligible for — but have not yet received — a PD-1 or PD-L1 inhibitor (a type of immunotherapy). Those therapies should be tried first when appropriate.
