Ezetimibe, Atorvastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the gist for your Liptruzet, which combines ezetimibe with atorvastatin. The atorvastatin part is cleared by a liver enzyme called CYP3A4. Lab and animal studies hinted that eleuthero might slow that enzyme down, which in theory could let atorvastatin build up a bit. When statin levels run higher, the things to watch for are muscle aches, weakness, or rarely more serious muscle problems.

The reassuring news: when researchers actually tested eleuthero (485 mg twice daily for two weeks) in healthy people, it did not meaningfully affect CYP3A4. So this concern stays mostly theoretical and unlikely. Still, if you start eleuthero and notice new muscle soreness, check in with your pharmacist or doctor.

Ezetimibe, Atorvastatin also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, eleuthero might decrease levels of drugs metabolized by OATP.
In vitro research suggests that eleuthero inhibits OATP2B1, which might reduce the bioavailability of oral drugs that are substrates of OATP2B1. Due to the weak inhibitory effect identified in this study, this interaction is not likely to be clinically significant.

Here's the short version: this is a very mild, mostly theoretical concern. Your Liptruzet combines two cholesterol medicines, and both rely partly on a gut transporter called OATP to get absorbed. In a lab dish, eleuthero slowed that transporter down a little. In theory, that could mean slightly less of the drug gets into your body, which could make it a touch less effective at lowering your LDL cholesterol.

But the effect seen in that study was weak, and it has not been shown to matter in actual people. I wouldn't lose sleep over it. If you'd like to use eleuthero regularly, just mention it to your pharmacist or doctor so they can keep an eye on your cholesterol numbers at your next check.
