Opicapone falls under P-GLYCOPROTEIN SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of P-glycoprotein substrates.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.

Here's the basic idea: opicapone is moved around your body in part by a transporter protein called P-glycoprotein. Eleuthero may slow that transporter down. If that happens, a little more opicapone could stay in your system, which in theory might nudge up its effects and side effects, such as more involuntary movements (dyskinesia), low blood pressure when standing, dizziness, or trouble sleeping. Opicapone is an active drug, not a prodrug, so blocking its clearance would tend to raise its levels, not lower them.

That said, this is based only on lab (test-tube) research, and it has never been reported in real people. So I wouldn't lose sleep over it. Just mention to your pharmacist or neurologist that you're considering eleuthero so they can keep an eye on you.

Ongentys (opicapone) is used alongside levodopa/carbidopa to treat Parkinson's disease in adults who experience 'off' episodes — periods when medication wears off and Parkinson's symptoms return before the next dose is due.

It is not used alone. Ongentys works as an add-on to help levodopa stay active longer, reducing the time patients spend in these 'off' states.
