Meperidine, Promethazine falls under SEROTONERGIC DRUGS — the group Ergot interacts with.
Serotonergic drugs include the selective serotonin reuptake inhibitors (SSRIs) such as fluoxetine (Prozac), paroxetine (Paxil), and sertraline (Zoloft); tricyclic and atypical antidepressants such as amitriptyline (Elavil), clomipramine (Anafranil), and imipramine (Tofranil); triptans such as sumatriptan (Imitrex), zolmitriptan (Zomig), and rizatriptan (Maxalt); opioids such as methadone (Dolophine) and tramadol (Ultram); and many other medications.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product pairs meperidine (a fairly strong opioid pain reliever) with promethazine (an antihistamine that helps with nausea). The piece that matters here is meperidine, which raises serotonin activity in the brain. Ergot alkaloids also nudge serotonin signaling, so on paper stacking the two could add up and push you toward serotonin syndrome, things like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion.
I want to be honest about the evidence: this is theoretical and based mostly on scattered case reports, not strong studies. Meperidine isn't a prodrug, so the concern is simply two serotonin-active things together. On top of that, ergot is genuinely toxic and shouldn't be used as a home supplement at all. Please run this by your pharmacist or doctor before combining them.

Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.
