Ergotamine, a constituent of ergot, is a substrate of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that inhibit CYP3A4 might increase the risk of ergot toxicity. Some of these drugs include amiodarone (Cordarone), clarithromycin (Biaxin), diltiazem (Cardizem), erythromycin (E-mycin, Erythrocin), indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase), and many others.

Here's the gist for anyone on saquinavir (Invirase): this HIV medicine is a strong blocker of a liver enzyme called CYP3A4. That same enzyme is what your body uses to break down ergot's active chemicals, like ergotamine. So when saquinavir slows that cleanup, ergot can build up to dangerous levels instead of being cleared out.

That matters because ergot already tightens blood vessels hard. Too much can cause cold, numb, painful hands and feet, bad leg cramps, severe headache, nausea, and in serious cases poor blood flow to fingers or toes. The evidence here is mostly theoretical and anecdotal, but the potential harm is real. Honestly, crude ergot isn't safe to take on its own anyway. Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.

What Is Saquinavir Used For?

  • Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
