Tucatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ergot interacts with.

Ergotamine, a constituent of ergot, is a substrate of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that inhibit CYP3A4 might increase the risk of ergot toxicity. Some of these drugs include amiodarone (Cordarone), clarithromycin (Biaxin), diltiazem (Cardizem), erythromycin (E-mycin, Erythrocin), indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase), and many others.

Here's the gist for these two. Tucatinib (Tukysa) is a strong blocker of an enzyme in your body called CYP3A4. Ergot chemicals like ergotamine are normally cleared by that same enzyme. So when CYP3A4 is slowed down, ergot can build up to higher levels than your body expects.

Why that matters: ergot already squeezes blood vessels hard, and too much can cause a dangerous reaction called ergotism (think severe headache, cold or numb fingers and toes, leg cramps, and poor circulation). Honestly, raw ergot isn't safe to take on its own anyway. The evidence here is theoretical and anecdotal, but the combo leans risky. Please don't use any ergot product without first checking with your oncology team or pharmacist.

Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.

What Is Tucatinib Used For?

Brand name: Tukysa

Tukysa (tucatinib) tablets are used in combination with trastuzumab and capecitabine to treat adults with advanced, unresectable, or metastatic HER2-positive breast cancer — including those whose cancer has spread to the brain — who have already received at least one HER2-targeted treatment.

Tukysa tablets are also used in combination with trastuzumab alone to treat adults with RAS wild-type, HER2-positive unresectable or metastatic colorectal cancer that has continued to grow after prior chemotherapy. This colorectal cancer use has received accelerated approval, meaning final approval depends on further study results confirming the benefit.
