This combination pill packs two ingredients that already make you drowsy: codeine (an opioid pain reliever) and phenobarbital (a barbiturate sedative). The concern here is simple. Berberine, the main active compound in European barberry, showed mild sedative effects in animal studies. So in theory, adding barberry on top of an already sedating medicine could pile on more sleepiness, slowed breathing, dizziness, or a foggy, slowed-down feeling.

I want to be honest about the evidence: this is theoretical, based on lab and animal work, not human reports, so I wouldn't panic. Still, with phenobarbital and codeine in the mix, more sedation isn't something to shrug off. Please check with your pharmacist or doctor before adding barberry, and never adjust this prescription on your own.

Theoretically, European barberry may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal evidence suggest that berberine, a constituent of European barberry, might inhibit platelet aggregation. Theoretically, European barberry might have a similar effect.

This combination product contains three medicines, but the part that matters here is the aspirin. Aspirin is an antiplatelet drug, meaning it makes your platelets less sticky so blood clots form more slowly. European barberry contains berberine, which in lab dishes and animal studies has also looked like it can slow platelets down a bit. Put the two together and, in theory, you could nudge that blood-thinning effect a little further.

I want to be honest about the evidence here: this is theoretical, based on test-tube and animal work, not real-world reports. Still, watch for things like easy bruising, nosebleeds, bleeding gums, or stomach upset, since aspirin can already irritate the stomach. Please run it past your pharmacist or doctor before adding a barberry supplement.
