Why this applies to Tadalafil
Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down tadalafil (Cialis). European barberry contains berberine, which lab research hints may slow that enzyme down. If it does, tadalafil could clear more slowly and build up a bit in your system. That can mean a stronger version of its usual effects, including headache, flushing, stuffy nose, back pain, indigestion, and possibly a noticeable drop in blood pressure with dizziness.
I want to be honest about the evidence here: this is theoretical, based on how the chemistry might behave, not on real-world reports in people. Tadalafil is not a prodrug, so a slowdown would make it stronger, not weaker. Still, this is mostly a caution flag. Before pairing barberry with tadalafil, check with your pharmacist or doctor.

What Is Tadalafil Used For?

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.
Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
