Aspirin, Caffeine, Dihydrocodeine Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with Synalgos DC: one of its ingredients, dihydrocodeine, is a prodrug. Your liver uses an enzyme called CYP2D6 to turn it into the active painkiller (dihydromorphine) that actually does the work. In lab tests, False Unicorn appears to slow that same enzyme down. If it does that in real life, your body might convert less of the dihydrocodeine, so you could get less pain relief than expected.

I want to be honest about the evidence here: this is based only on test-tube work, not on people, so it's theoretical and may not amount to much. The aspirin and caffeine parts aren't really affected. Still, before mixing the two, please check with your pharmacist or doctor so they can guide you safely.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the thing about Synalgos DC: the dihydrocodeine part is a bit different from a simple painkiller. Like codeine, dihydrocodeine is partly a prodrug, meaning your body has to convert it into its more active, stronger pain-relieving form. CYP3A4 is one of the enzymes involved in handling this opioid. False Unicorn has been shown, in a test tube only, to slow CYP3A4 down. In real life, that could nudge dihydrocodeine levels a little higher, which might mean more drowsiness, constipation, nausea, or slowed breathing.

That said, this is very weak, lab-only evidence, so I wouldn't lose sleep over it. The caffeine and aspirin pieces aren't a big concern here. Still, since opioids deserve respect, please run this combo by your pharmacist or doctor first.
