Erythromycin Gluceptate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for you. Erythromycin is partly cleared by a liver enzyme called CYP3A4. In a lab dish (not in people), false unicorn appeared to slow that same enzyme down. The thinking is that if it does the same thing in your body, your erythromycin might not break down as quickly and could build up a bit higher than usual.

With erythromycin specifically, higher levels can mean more stomach upset, nausea, cramping, and (importantly) a small chance of affecting your heart rhythm (QT prolongation). I want to be honest though: this is only a theoretical, test-tube concern, not something proven in real patients. Still, since safety data on false unicorn is thin, please run this combination by your pharmacist or doctor before mixing them.
