Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with hydrocodone. Your body uses an enzyme called CYP2D6 to convert hydrocodone into hydromorphone, which is the form that does much of the pain-relieving work. So hydrocodone acts a bit like a prodrug. In a lab dish, false unicorn appears to slow down CYP2D6. If that happened in real people, it could mean less of that active form, so you might get weaker pain relief than usual.

I want to be honest about how thin the evidence is. This is only a test-tube finding, with no human studies, so we can't say it truly matters in everyday life. Still, if you take hydrocodone for ongoing severe pain, it's worth checking with your pharmacist or doctor before adding false unicorn rather than changing anything yourself.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for hydrocodone. Your body relies partly on a liver enzyme called CYP3A4 to break this opioid down and move it out of your system. In lab (test tube) studies, False Unicorn appears to slow that same enzyme down. The worry is that if hydrocodone gets cleared more slowly, more of it could linger in your blood, which might mean stronger effects than expected: think extra drowsiness, dizziness, constipation, or slowed breathing.

I want to be honest with you, though. This is only seen in lab testing, not in real people, so it's more of a theoretical caution than a proven problem. Still, opioids aren't something to gamble with. Please run it by your pharmacist or doctor before combining the two.
