Why this applies to Hydrocodone, Ibuprofen

Hydrocodone, Ibuprofen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with Vicoprofen (the hydrocodone plus ibuprofen combo): hydrocodone is partly a prodrug. Your liver uses an enzyme called CYP2D6 to convert some of it into hydromorphone, which gives a chunk of the pain relief. In a lab dish, False Unicorn appeared to block CYP2D6. If that happened in the body, your liver might make a little less of that active form, so the pain relief could feel weaker. The picture is muddy, though, because hydrocodone also works directly on its own.

I want to be honest: this is only test-tube data, not proven in people, so I wouldn't lose sleep over it. The ibuprofen side isn't involved here at all. Before mixing the two, just run it by your pharmacist or doctor.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with Vicoprofen, which pairs hydrocodone (an opioid) with ibuprofen (an anti-inflammatory). In a test tube, False Unicorn appears to slow down CYP3A4, one of the liver enzymes that helps clear the hydrocodone portion of this pill. If that enzyme runs slower, hydrocodone could linger a bit longer than usual, which in theory might nudge up its effects, things like extra drowsiness, dizziness, constipation, or in a worst case slowed breathing.

I want to be honest about how thin this evidence is. It comes only from lab work, not from real people, so we genuinely don't know if it matters in everyday life. Still, since opioids carry real risk, it's worth a quick chat with your pharmacist or doctor before combining the two.
