Pitolisant Hydrochloride falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist for Wakix (pitolisant): your body relies on a liver enzyme called CYP2D6 to help clear this medication. In lab testing (test tube only, not in people), False Unicorn appeared to slow that same enzyme down. If that held true in the body, pitolisant could clear a bit more slowly and build up, which might mean stronger effects or more side effects like headache, trouble sleeping, nausea, anxiety, or a faster heartbeat.

I want to be honest about the evidence though: this is theoretical and based only on a lab study, so we don't really know if it matters in real life. Since pitolisant is a fairly specialized drug, I'd just run this combo past your pharmacist or doctor before mixing the two.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the situation with Wakix (pitolisant) and False Unicorn. Your body uses an enzyme called CYP3A4 to help clear pitolisant out of your system. In lab studies (test tube only, not in people), False Unicorn seemed to slow that enzyme down. In theory, a slower enzyme means pitolisant could build up a bit higher than usual.

If that happened, you might notice more of pitolisant's typical effects, like trouble sleeping, headache, nausea, anxiety, or a faster heartbeat. I want to stress this is theoretical and based only on weak lab evidence, so I'm not telling you to panic or change anything. Just loop in your pharmacist or doctor before adding False Unicorn so they can keep an eye on how you feel.

What Is Pitolisant Used For?

Brand name: Wakix

Wakix (pitolisant) is used to treat two hallmark problems of narcolepsy: excessive daytime sleepiness (feeling overwhelmingly tired and unable to stay awake during the day) and cataplexy (sudden episodes of muscle weakness or loss of muscle control, often triggered by emotion). It is approved for adults and children 6 years of age and older.

Both uses are specific to Wakix tablets taken by mouth. The drug is not approved for sleepiness or cataplexy caused by conditions other than narcolepsy.
