Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.

Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Here's the gist for aliskiren (Tekturna). Lab studies hint that fennel might slow down a liver enzyme called CYP3A4, which helps clear aliskiren from your body. If that enzyme is dampened, aliskiren could build up a bit and push its effect harder than expected. With a blood pressure medicine, that might mean your pressure drops too low, leaving you dizzy, lightheaded, or tired. Aliskiren is not a prodrug, so more drug simply means a stronger effect, not a weaker one.

The reassuring part: this has only been seen in test tubes, never in actual people, so the worry is mostly theoretical. Everyday fennel in cooking is very unlikely to matter. Before using concentrated fennel supplements or the oil, just check with your pharmacist or doctor.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
