Why this applies to Elexacaftor, Tezacaftor, Ivacaftor
Elexacaftor, Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group German Chamomile interacts with.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
All three pieces of Trikafta (elexacaftor, tezacaftor, and ivacaftor) are handled by a liver enzyme called CYP3A4. German chamomile has shown, in lab dish studies only, that it might slow that enzyme down. In theory, if chamomile blocks CYP3A4, your body could clear Trikafta a little slower and levels might creep up. That could mean more of its usual side effects, things like headache, belly pain, diarrhea, rash, or changes in liver-related blood tests.
Here's the reassuring part: this has never actually been seen in people, and an occasional cup of chamomile tea is unlikely to be a problem. Concentrated chamomile supplements are more of a question mark. Before you make this a daily habit, check with your CF team or pharmacist so they can keep an eye on things.

Trikafta (elexacaftor, tezacaftor, and ivacaftor) is used to treat cystic fibrosis (CF) in adults and children aged 2 years and older who have a clinical diagnosis of CF and carry at least one CFTR gene variant that is expected to respond to the medication — either based on clinical trial data or laboratory testing.
If a patient's genetic variant is not already known, an FDA-cleared CF genetic test should be used to confirm eligibility before starting treatment.
