Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group German Chamomile interacts with.
In vitro evidence shows that German chamomile might inhibit CYP2C9. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP2C9 in patients taking German chamomile.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the plain version for Opsynvi, which combines macitentan and tadalafil. Neither of these is a prodrug, so they work directly once absorbed. The concern on file is that German chamomile might block a liver enzyme called CYP2C9, which helps clear certain medicines. If that happens, a little more of the drug could hang around in your body than expected.
In practice, more tadalafil could mean more headache, flushing, stuffy nose, or low blood pressure, while extra macitentan could add to that dizziness or fluid buildup. But I want to be honest: this is theoretical, based only on lab tests, not real people. Everyday cups of chamomile tea are unlikely to cause trouble. Concentrated capsules are more of a question mark, so check with your pharmacist or doctor first.

In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.

Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared from your body largely by a liver enzyme called CYP3A4. German chamomile has shown, in lab dish studies only, that it might slow this enzyme down. If that happened in real life, these drugs could build up a bit more than usual, which might mean more of their typical effects like low blood pressure, dizziness, headache, flushing, or a stuffy nose.
Here is the reassuring part: this is only theoretical. It has never actually been reported in people, and an occasional cup of chamomile tea is very unlikely to cause trouble. Concentrated chamomile supplements are a bigger question mark. Since Opsynvi treats a serious condition, just run any regular chamomile use past your pharmacist or doctor first.
