German chamomile has mild sedative effects. Theoretically, concomitant use with drugs with sedative properties can cause additive effects and side effects.

Here's the thing to keep in mind with Tramacet (which combines tramadol with acetaminophen): the tramadol part already has its own calming, drowsy effect on your brain. German chamomile is thought to be mildly sedating too, mostly because of a compound called apigenin that gently nudges the same relaxation pathways. Put them together and those effects may simply add up.
In plain terms, you might feel sleepier, foggier, or more lightheaded than the medicine alone would cause. I'll be honest, this concern is mostly theoretical and comes from lab studies, not strong human data, and a cup of chamomile tea is far milder than concentrated capsules. Still, go slow and check with your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro evidence shows that German chamomile might inhibit CYP2D6. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP2D6 in patients taking German chamomile.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's a wrinkle worth knowing about Tramacet. The tramadol part is actually a prodrug, meaning your body has to convert it (using a liver enzyme called CYP2D6) into the form that gives most of the pain relief. German chamomile may gently slow that same enzyme down. In theory, that could leave you with a bit more of the parent tramadol but less of the strong painkilling form, so your pain relief might not feel as complete. At the same time, leftover tramadol could nudge up side effects like drowsiness, dizziness, or nausea.
The honest part: this is based on lab studies only, not real people, so it may not amount to much, especially with an occasional cup of chamomile tea. Still, if you use chamomile regularly or notice changes, check in with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Tramacet, which pairs tramadol with acetaminophen. Lab studies hint that German chamomile may gently slow down a liver enzyme called CYP3A4, one of the pathways that helps process tramadol. If that pathway slows, a bit more tramadol could linger in your system. Tramadol is also a bit unusual: it relies on enzymes to turn into its more active form, so the net effect on pain relief isn't perfectly predictable, but you could notice more of its typical effects like drowsiness, dizziness, nausea, or constipation.
The important part: this is only seen in test-tube research, not in real people, so for most folks an occasional cup of chamomile tea is unlikely to cause trouble. Still, if you take chamomile regularly or in concentrated supplement form, it's worth a quick chat with your pharmacist or doctor.

Here's the honest picture on combining chamomile tea with Tramacet (tramadol plus acetaminophen). The concern on file is that chamomile may slow down a liver enzyme called CYP1A2. The thing is, this enzyme isn't a major player in clearing either tramadol or acetaminophen, so the practical impact here is likely very small.
On top of that, this is only a theoretical worry based on lab and animal studies, not anything actually seen in people. A cup or two of chamomile tea is very unlikely to cause trouble. If you used concentrated chamomile supplements daily, it's worth a quick chat with your pharmacist, mostly to keep an eye out for extra drowsiness, since both chamomile and tramadol can make you sleepy.
