Ginger and Glasdegib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Glasdegib

Glasdegib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginger interacts with.

Ginger might increase or decrease the levels of CYP3A4 substrates.
In vitro research and some case reports suggest that ginger inhibits CYP3A4 activity. Three case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking ginger and cancer medications that are CYP3A4 substrates (imatinib, dabrafenib, and crizotinib). However, the causality of this interaction is unclear due to the presence of multiple interacting drugs and routes of administration.
Conversely, other in vitro research suggests that ginger induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture with glasdegib (Daurismo) and ginger. Your body clears glasdegib using a liver enzyme called CYP3A4. Ginger may tug on that enzyme, but the lab data points in both directions. If ginger slows the enzyme, glasdegib could build up, which might mean more muscle cramps, nausea, fatigue, or changes in your heart rhythm (this drug can affect the QT interval). If ginger speeds the enzyme up instead, glasdegib might work less well against the leukemia.

The key thing: this is based on test-tube and animal studies, not solid human proof, so we don't truly know what happens. Small culinary amounts of ginger in food are very unlikely to matter. Before using ginger supplements during AML treatment, please run it by your oncology team or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Glasdegib page

“Grapefruit is the first thing I would take off the table for anyone on glasdegib (Daurismo), a targeted pill for acute myeloid leukemia. It can push glasdegib levels up because the drug is cleared by the CYP3A4 enzyme, and 121 of the 146 ingredients touch that pathway, some raising its level and others, like St.”

About Glasdegib

What Is Glasdegib Used For?

Brand name: Daurismo

Daurismo (glasdegib) is approved to treat newly-diagnosed acute myeloid leukemia (AML) — a fast-growing cancer of the blood and bone marrow. It is used specifically in adults who are 75 years of age or older, or in adults of any age whose other health conditions make intensive induction chemotherapy too risky.

Daurismo is not used alone. It is always given together with low-dose cytarabine, a separate chemotherapy medicine, as part of a 28-day treatment cycle.
