Why this applies to Elbasvir, Grazoprevir

Elbasvir, Grazoprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP3A4.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

Here's the concern with combining Ginkgo and Zepatier (elbasvir/grazoprevir). Both of these antiviral ingredients are partly handled by a liver and gut enzyme called CYP3A4. There's some thought that Ginkgo might speed up (induce) this enzyme. If that happens, your body could clear the medication a little faster, which means lower drug levels. For a hepatitis C treatment, that matters, because keeping the drug levels high enough is what helps fully clear the virus. Weaker levels could risk a less successful course of treatment.

That said, the evidence here is honestly mixed and mostly theoretical. Some studies showed Ginkgo didn't meaningfully change similar drugs. I wouldn't panic, but since finishing hepatitis C therapy successfully is so important, please check with your pharmacist or doctor before adding Ginkgo.

About Elbasvir and Grazoprevir

What Is Elbasvir and Grazoprevir Used For?

Brand name: Zepatier

Zepatier is used to treat chronic hepatitis C virus (HCV) genotype 1 or genotype 4 infection in adults and children 12 years of age and older (or weighing at least 30 kg). It works against both the most common strain of HCV (genotype 1) and genotype 4.

Depending on the specific genotype, prior treatment history, and certain viral characteristics, Zepatier may be prescribed alone or combined with another antiviral medicine called ribavirin. Your prescriber will determine the right regimen and duration for your situation.
