tipranavir falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Ginkgo interacts with.
Some clinical research shows that a specific ginkgo leaf extract (Remembrance, Herbs Product LTD) 140 mg twice daily can induce CYP2C19 enzymes and potentially decrease levels of drugs metabolized by these enzymes. However, other clinical research shows that taking ginkgo 120 mg twice daily for 12 days has no effect on levels of drugs metabolized by CYP2C19.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with pairing ginkgo and tipranavir (Aptivus). Tipranavir is an active HIV protease inhibitor, not a prodrug, so it works as-is once you take it. Some research suggests ginkgo can speed up a liver enzyme called CYP2C19, and tipranavir is partly handled by that enzyme. If ginkgo revs that enzyme up, your body could clear tipranavir a little faster and your blood levels might dip.
With HIV medicine, lower-than-needed levels are the worry, because that can let the virus rebound or build resistance over time. That said, this is mostly theoretical, the evidence is mixed (one study showed no effect), and HIV regimens are tightly managed. Please don't stop or change anything on your own. Just check with your pharmacist or HIV doctor before adding ginkgo so they can keep your treatment on track.

Laboratory research suggests that ginkgo leaf extract can mildly inhibit CYP1A2 enzymes. However, clinical research suggests ginkgo might not affect CYP1A2. Until more is known, use ginkgo cautiously in patients taking drugs metabolized by these enzymes.

Here's the short version on combining ginkgo with tipranavir (Aptivus). The concern on file is that ginkgo might mildly slow down a liver enzyme called CYP1A2, which in theory could let drugs cleared by that enzyme build up a bit. If tipranavir levels edged up, you might see more of its usual side effects, like nausea, diarrhea, tiredness, or changes in your liver tests.
That said, this is mostly theoretical. Lab studies hint ginkgo could block CYP1A2 a little, but actual human studies suggest it may not matter much, and tipranavir is handled mainly by other pathways. The bigger real-world point: ginkgo can thin the blood, and HIV therapy is too important to risk. Please loop in your pharmacist or doctor before adding ginkgo.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro, a specific standardized extract of ginkgo leaf (EGb 761) inhibits CYP2C9 activity . The terpenoid (ginkgolides) and flavonoid (quercetin, kaempferol, etc.) constituents seem to be responsible for this effect. Most ginkgo extracts contain some amount of these constituents. Therefore, other ginkgo leaf extracts might also inhibit the CYP2C9 enzyme. However, clinical research suggests that ginkgo might not have a significant effect on CYP2C9 in humans. Ginkgo does not seem to significantly affect the pharmacokinetics of CYP2C9 substrates diclofenac or tolbutamide.

Here's the short version: in lab dishes, ginkgo can slow down an enzyme called CYP2C9, which is one of the pathways your body uses to clear certain medicines. The thinking is that if that pathway is blocked, a drug it handles could build up a bit and cause stronger effects. Tipranavir (Aptivus) is an active drug, not a prodrug, so in theory higher levels could mean more of its usual side effects, like nausea, diarrhea, or stress on the liver.
That said, this concern comes only from test-tube work, and in actual human studies ginkgo did not meaningfully change CYP2C9-cleared drugs. So the real-world risk looks low. With an HIV medicine, though, I'd still want you to keep blood levels steady, so please check with your pharmacist or HIV provider before adding ginkgo. Don't change anything on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

Tipranavir is cleared in part by an enzyme called CYP3A4, and it is taken with ritonavir to keep its blood levels high enough to fight HIV. The worry here is theoretical: some research suggests ginkgo could speed up this enzyme, which might lower tipranavir levels over time. If that happened, the virus could get a foothold again and you could lose viral control, the same concern we saw in a couple of case reports with another HIV drug, efavirenz.
That said, the evidence is mixed and far from settled, and ginkgo did not seem to lower ritonavir in early studies. Still, with HIV therapy I lean cautious. Please check with your pharmacist or doctor before adding ginkgo, and don't change your medication on your own.

P-GLYCOPROTEIN SUBSTRATES

A small clinical study in healthy volunteers shows that using ginkgo leaf extract 120 mg orally three times daily for 14 days can increase levels of the P-glycoprotein substrate, talinolol, by 36% in healthy male individuals. However, single doses of ginkgo do not have the same effect.

Here's the situation with tipranavir (Aptivus) and ginkgo. Your body uses a kind of cellular pump called P-glycoprotein to move tipranavir around and out. Ginkgo may slow that pump down a bit, and when the pump works slower, more of the drug can stay in your blood. In plain terms, ginkgo could nudge tipranavir levels up, which might mean more of its usual side effects like nausea, diarrhea, stomach upset, or stress on your liver.
I want to keep this in perspective. This concern is mostly theoretical, based on a small study using a different drug, and single doses of ginkgo didn't cause it. Still, HIV therapy is too important to guess with, so please check with your pharmacist or doctor before adding ginkgo. Don't change your tipranavir on your own.

Ginkgo is one of the world's most popular herbal supplements, mostly taken to support memory and circulation. What Is Tipranavir Used For?
Brand name: Aptivus
Aptivus (tipranavir) is used to treat HIV-1 infection in adults and in pediatric patients weighing at least 36 kg (about 79 lbs). It is specifically for people who have already tried other HIV treatments and whose virus has become resistant to more than one protease inhibitor. It is always taken together with a low dose of ritonavir and with other HIV medications.
Aptivus is not recommended for people who have never taken HIV treatment before (treatment-naïve patients). Using tipranavir alongside additional active HIV drugs improves the chances of it working well.
