Metformin, Linagliptin falls under ANTIDIABETES DRUGS — the group Guava interacts with.

Theoretically, concomitant use with antidiabetes drugs might have additive effects and increase the risk of hypoglycemia. Animal research shows that guava leaf extract or guava fruit can have hypoglycemic effects. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, metformin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the short version: your Jentadueto already does two jobs to lower blood sugar, since it pairs metformin with linagliptin. Guava leaf, in animal and lab studies, has shown its own mild blood-sugar-lowering effect. So the worry is simple addition, two things pushing your glucose down at the same time, which could nudge you toward a low (hypoglycemia).

That said, this is theoretical and based on early animal research, not solid human studies, so I wouldn't panic. Eating guava as a fruit is very different from taking concentrated guava leaf supplements, which are the bigger concern. If you do try a guava leaf product, watch for shakiness, sweating, dizziness, or confusion, keep an eye on your readings, and check with your pharmacist or doctor first.
