These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Guggul interacts with whole categories of medication. Aspirin, Butalbital, Caffeine falls into 2 of those categories, so it can interact with Guggul in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Aspirin, Butalbital, Caffeine
Aspirin, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist with Fiorinal (aspirin, butalbital, and caffeine) and guggul. Lab and animal studies hint that guggul may speed up a liver enzyme called CYP3A4, which helps break down the butalbital part of this combo. If that enzyme works faster, your body could clear the butalbital a bit quicker, which in theory might make it less effective at easing your tension headache.
I want to be honest about how thin this evidence is. It comes only from test-tube and animal research, not real patients, so it's more of a theoretical concern than a proven problem. Still, since guggul can also upset your stomach (and aspirin can too), it's worth running it by your pharmacist or doctor before combining them. Don't stop or change anything on your own.

ANTICOAGULANT/ANTIPLATELET DRUGS
2 of 2Aspirin, Butalbital, Caffeine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.
In vitro research and preliminary clinical studies suggest that guggul might have antiplatelet and anticoagulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with Fiorinal: one of its ingredients is aspirin, which already thins your blood a bit by making platelets less sticky. Guggul may have its own mild blood-thinning and antiplatelet action, based on lab work and a few small, lower-quality studies. So stacking the two could, in theory, nudge your bleeding risk up a little.
In real life that might look like easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop. This is more of a theoretical, take-it-seriously-but-don't-panic caution than a proven danger. Still, if you take Fiorinal regularly or have surgery coming up, check in with your pharmacist or doctor before adding a guggul supplement.

“The aspirin inside Fiorinal (aspirin, butalbital, caffeine) is the reason this list deserves careful reading rather than a quick skim.
