Cilostazol falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Holy Basil interacts with.

Theoretically, holy basil seed oil might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Animal research shows that holy basil seed oil can prolong bleeding time, possibly due to inhibition of platelet aggregation. However, it is not known if this occurs in humans.

Here's the gist: cilostazol is an antiplatelet drug, which means it already makes your platelets a little less sticky to improve blood flow in your legs. Holy basil seed oil may do something similar. In animal studies it lengthened bleeding time, possibly by quieting platelets too. Stack the two together and, in theory, you could nudge your bleeding risk up a bit.

I want to be honest about the evidence here, though: this is based on lab and animal work only, and we don't actually know if it happens in people. So this is more of a 'keep an eye out' than a red alert. Watch for easy bruising, nosebleeds, or bleeding gums, and check with your pharmacist or doctor before adding holy basil while you're on Pletal.
