Theoretically, holy basil seed oil might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Animal research shows that holy basil seed oil can prolong bleeding time, possibly due to inhibition of platelet aggregation. However, it is not known if this occurs in humans.

Cangrelor (Kengreal) is a powerful IV antiplatelet medicine given in the hospital, usually during a cardiac procedure like a stent placement, to keep platelets from clumping and forming clots. Holy basil may add a little to that same effect. In animal studies, holy basil seed oil lengthened bleeding time, possibly by making platelets less sticky too. Stacked together, the theoretical concern is a bit more bleeding or bruising.

Here's the reassuring part: this is based on lab and animal data, not proven in people, so I wouldn't lose sleep over it. Also, cangrelor is given by your hospital team who watch you closely. Still, always tell them about any supplements you take, including tulsi, so nothing catches them off guard.
