Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Holy Basil interacts with.

Theoretically, holy basil seed oil might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Animal research shows that holy basil seed oil can prolong bleeding time, possibly due to inhibition of platelet aggregation. However, it is not known if this occurs in humans.

Here's the thing to keep in mind with iloprost (Ventavis): besides relaxing the blood vessels in your lungs, it also makes platelets a little less sticky, which can nudge your bleeding risk up on its own. Holy basil seed oil has been shown, but only in animal studies, to do something similar by slowing platelet clumping. So the worry is that stacking the two could add up and make you bruise more easily or bleed a bit longer than usual.

I want to be honest that this is a theoretical concern. We don't have human data showing it actually happens, and culinary tulsi tea is far weaker than a concentrated seed oil supplement. Still, before you add holy basil, run it by your pharmacist or doctor so they can keep an eye on any bleeding or bruising.
