Ibrutinib falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Horse Chestnut interacts with.

Horse chestnut may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Horse chestnut contains the constituent esculin which has been shown to have antithrombotic effects. Therefore, horse chestnut might have antiplatelet effects. This has not been shown in humans.

Here's the thing to keep in mind with ibrutinib (Imbruvica): this cancer medicine already carries a known bleeding risk on its own. It works partly by blocking a signal (BTK) that platelets use to clot, so people on it can bruise more easily or bleed longer than usual. Horse chestnut contains a compound (esculin) that lab work suggests may also have a mild blood-thinning, anti-clotting effect.

So in theory, pairing the two could nudge your bleeding risk a little higher, meaning more bruising, nosebleeds, bleeding gums, or harder-to-stop cuts. I want to be honest though: this anti-platelet effect from horse chestnut has not been proven in people, so it's a cautionary, theoretical concern. Please check with your oncologist or pharmacist before adding it.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is approved for adult patients with chronic lymphocytic leukemia (CLL) / small lymphocytic lymphoma (SLL), including those with a specific genetic change called 17p deletion, and for adult patients with Waldenström's macroglobulinemia (WM), a rare type of blood cancer.

Imbruvica is also approved for chronic graft-versus-host disease (cGVHD) — a complication that can occur after a stem cell transplant — in adults and children aged 1 year and older whose condition did not respond to at least one prior treatment.
