Why this applies to Duvelisib

Duvelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Your body relies on an enzyme called CYP3A4 to break down duvelisib (Copiktra) and clear it from your system. Hu Zhang contains resveratrol, which in lab studies may slow that same enzyme down. In theory, that could let duvelisib build up a bit higher than expected, and since duvelisib is an active drug (not a prodrug), more of it could mean stronger effects and a higher chance of side effects like diarrhea, infections, low blood counts, liver problems, or lung inflammation.

That said, this is a theoretical concern based mostly on test-tube work, and one human study using high-dose resveratrol didn't force any dose changes. Still, because duvelisib is a serious cancer medicine, please run any Hu Zhang use by your oncologist or pharmacist first.

About Duvelisib

What Is Duvelisib Used For?

Brand name: Copiktra

Copiktra (duvelisib) is used in adults with relapsed or refractory chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) — meaning the cancer has returned or stopped responding to treatment — after at least two prior therapies.

Copiktra is also used for relapsed or refractory follicular lymphoma (FL) after at least two prior systemic therapies. The follicular lymphoma approval is based on overall response rate and is considered an accelerated approval; continued approval may depend on results from confirmatory trials. Copiktra is not intended for use as a first- or second-line treatment for any of these cancers.
