Why this applies to Entrectinib

Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down and clear entrectinib (Rozlytrek). The resveratrol in Hu Zhang may, at least in lab studies, slow that enzyme down a bit. If it does, entrectinib could clear more slowly and build up in your blood, which can mean stronger effects and a higher chance of side effects like dizziness, fatigue, swelling, constipation, changes in heart rhythm, or numbness and tingling.

I want to keep this in perspective, though. This concern is theoretical, based on test-tube data, and one human study using high-dose resveratrol didn't force any dose changes. Still, entrectinib is a serious cancer drug, so please don't add Hu Zhang without checking with your oncologist or pharmacist first.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
