Theoretically, hu zhang might increase levels of drugs metabolized by CYP1A2.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.

Here's the gist with Ortho-Prefest (estradiol plus norgestimate) and Hu Zhang. The resveratrol in this herb can, at least in a test tube, slow down a liver enzyme called CYP1A2, which helps clear the estrogen part of your hormone therapy. If that pathway slows, estrogen levels could theoretically drift a bit higher. That might mean more of the usual estrogen-related effects, like breast tenderness, bloating, nausea, headaches, or breakthrough spotting.

The important part: this is only a theoretical concern based on lab work, and it hasn't actually been seen in people. So I wouldn't panic. Still, before you start a concentrated Hu Zhang supplement alongside your hormones, run it by your pharmacist or doctor so you can keep an eye on how you feel.

Ortho-Prefest combines estradiol (an estrogen) with norgestimate (a progestin), and it's often used to ease menopause symptoms like hot flashes. Hu Zhang is rich in resveratrol, which has weak estrogen-like activity in the test tube. The thinking is that resveratrol could compete with your prescribed estrogen at its receptors, possibly blunting how well your therapy works.

I want to be straight with you though: this concern comes only from lab and animal studies, not from real people taking both together. So it's more theoretical than proven. Still, if you noticed your hot flashes or other symptoms creeping back after adding Hu Zhang, that would be worth mentioning. Please loop in your doctor or pharmacist before starting it, rather than changing anything on your own.
