Here's the gist for Inrebic (fedratinib). Your body uses a liver enzyme called CYP2C19 to help clear this medicine. Hu Zhang contains resveratrol, which in lab dishes has slowed that enzyme down. If that same effect happened in people, fedratinib could build up a bit higher than expected. That might mean more of its usual side effects, such as nausea, diarrhea, tiredness, or changes in your blood counts and liver tests.

I want to be honest about how shaky this is. It's a theoretical concern based only on test-tube work, and it has never actually been reported in real patients. Still, fedratinib is a serious cancer drug with careful monitoring, so don't add Hu Zhang on your own. Run it by your oncology team or pharmacist first.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the basic idea with fedratinib (Inrebic). Your body clears this medicine largely through a liver enzyme called CYP3A4. Hu Zhang contains resveratrol, which in lab studies can slow that same enzyme down. If that happened in a real person, fedratinib could build up a bit higher than expected, which might mean more of its usual side effects like nausea, diarrhea, fatigue, or low blood counts. Fedratinib is an active drug, not a prodrug, so blocking the enzyme would tend to raise its effect, not weaken it.

The good news: this is theoretical, based on test-tube data, and one human study didn't see meaningful problems. Still, since fedratinib is a serious cancer medicine, please check with your oncology team or pharmacist before adding Hu Zhang.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
