Hu Zhang + Mirtazapine
Mirtazapine brand name: Remeron.
Mirtazapine falls into 2 drug categories that Hu Zhang can interact with; the most serious is rated moderate. Theoretically, hu zhang might increase levels of drugs metabolized by CYP1A2.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Hu Zhang interacts with whole categories of medication. Mirtazapine falls into 2 of those categories, so it can interact with Hu Zhang in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Mirtazapine
Mirtazapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hu Zhang interacts with.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses an enzyme called CYP1A2 (along with a few others) to help break down mirtazapine. Hu Zhang contains resveratrol, and in test-tube studies that compound appears to slow this enzyme down. If it slowed it in your body too, mirtazapine could clear a little more slowly and build up. That might mean stronger effects than usual, things like extra drowsiness, more grogginess in the morning, dry mouth, dizziness, or increased appetite and weight gain.
The reassuring part: this is purely theoretical, based on lab work, and has never actually been reported in people. Mirtazapine is mostly active on its own (not a prodrug), so the worry is more, not less, sedation. Still, if you want to combine them, run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Mirtazapine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for mirtazapine (Remeron). Your body uses several liver enzymes to clear this medicine, and one of them is CYP3A4. The resveratrol in Hu Zhang has shown, in test-tube studies, that it might slow CYP3A4 down a little. If that happened in a real person, mirtazapine could build up slightly and you might feel more of its usual effects, like extra drowsiness, dizziness, increased appetite, or grogginess the next morning.
That said, this is mostly a theoretical concern. In one human study, even high-dose resveratrol didn't force any dose changes for CYP3A4 medicines. So I wouldn't be alarmed, but it's worth a quick chat with your pharmacist or doctor before adding Hu Zhang, especially if mirtazapine already makes you sleepy.

From our pharmacist’s overview on the Mirtazapine page
“Depression and poor sleep tend to travel together, which is why people on mirtazapine (Remeron) so often eye herbal mood and sleep remedies, and that overlap is exactly where this list demands real care. Four of the 212 ingredients on file carry a major rating.”

About Hu Zhang
What is Hu Zhang?
- Source of resveratrol
- Heart and circulation support
- Inflammation
- Liver and digestive complaints (traditional)
- Cough and respiratory symptoms (traditional)
- Skin and joint conditions (traditional topical use)
Hu Zhang (Japanese knotweed root) is a traditional Chinese herb that is one of the richest natural sources of resveratrol and emodin. Some lab and early human research looks interesting for inflammation and heart health, but high-quality evidence in people is limited. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
About Mirtazapine
What Is Mirtazapine Used For?
Brand name: Remeron
Mirtazapine tablets, mirtazapine orally disintegrating tablets, Remeron, and RemeronsолTab are all indicated for the treatment of major depressive disorder (MDD) in adults. Major depressive disorder involves a persistently low or sad mood most days, often accompanied by loss of interest, sleep changes, fatigue, and difficulty concentrating.
The effectiveness of mirtazapine tablets in maintaining a response for up to 40 weeks after an initial treatment period has also been demonstrated. Patients should be reassessed periodically to determine whether continued treatment is needed.
