Mometasone, formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.

Here's the honest picture with Zenhale (mometasone and formoterol) and Hu Zhang. The concern on file is that resveratrol, one of the main compounds in this herb, might slow down a liver enzyme called CYP2C19. In theory, that could let certain medicines build up a bit. The catch is that this is all lab-dish research, and it has never actually been seen in people taking these two together.

On top of that, your inhaler is breathed straight into your lungs, so very little reaches the liver to be processed this way. Realistically, I'd expect little to no meaningful change. If you did notice things like a faster heartbeat, jitteriness, or shakiness from the formoterol, mention it. Just loop in your pharmacist or doctor before adding Hu Zhang so they can okay it for you.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Your Zenhale inhaler combines two medicines, and both mometasone (the steroid) and formoterol (the long-acting breathing opener) are partly broken down by a liver enzyme called CYP3A4. The thinking here is that resveratrol in Hu Zhang could slow that enzyme down a little, which in theory might let slightly more of these drugs build up in your body.

If that happened, you might notice a bit more of formoterol's known effects, like a faster or pounding heartbeat, jitteriness, or shakiness, and more steroid exposure over time. But I want to be honest: this is theoretical, based on lab data. In a human study, even 3000 mg of resveratrol daily did not force any dose changes. With an inhaled medicine, the amounts reaching your blood are small. Still, run it past your pharmacist before combining them.
