Tapentadol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.

Here's the gist with Hu Zhang and Tapentadol. Hu Zhang is rich in resveratrol, and in test-tube studies resveratrol can slow down a liver enzyme called CYP2C19. Tapentadol is partly broken down by that enzyme, so in theory blocking it could let a little more tapentadol linger in your system. If that happened, you might notice the usual opioid effects turning up a notch: more drowsiness, dizziness, nausea, constipation, or feeling foggy.

I want to be honest about the strength here, though. This is a theoretical concern based on lab work only, and it has never actually been reported in people. Tapentadol also leans more on a different pathway (glucuronidation) for most of its clearance, so any real effect is likely small. Still, since this is an opioid, please run it by your pharmacist or doctor before combining them.

About Tapentadol

What Is Tapentadol Used For?

Brand names: Nucynta, Nucynta ER

Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.

Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
