Theoretically, taking iboga with serotonergic drugs might increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.

Let me put this one in plain terms. This combo product mixes a pain reliever (acetaminophen), an antihistamine (chlorpheniramine), and a decongestant (pseudoephedrine). The piece that matters here is that both chlorpheniramine and pseudoephedrine can nudge serotonin activity upward. Iboga (through ibogaine) does the same thing, so the worry is an additive serotonin effect when they overlap.

Honestly, this is a theoretical concern based on how these drugs work, not strong real-world reports. Still, if serotonin climbs too high you could feel agitation, a racing heart, sweating, shivering, or shakiness. Ibogaine also carries its own serious heart-rhythm risks, and pseudoephedrine can raise blood pressure too. Please don't combine these without first checking with your pharmacist or doctor.

ANTICHOLINERGIC DRUGS

Theoretically, iboga might decrease the effectiveness of anticholinergic drugs.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.

The part of this combo product that matters here is chlorpheniramine, the antihistamine. It works partly by quieting the chemical messenger acetylcholine. Lab studies suggest Iboga might raise acetylcholine levels, which in theory could push against chlorpheniramine and make it a bit less effective. So instead of stacking up, your allergy relief might be slightly weaker, with more sneezing, itching, or runny nose getting through.

Honestly though, this is only test-tube data, and the main active piece of Iboga (ibogaine) did not seem to cause this effect. So I would not lose sleep over the allergy part. The far bigger concern is Iboga itself, which can cause dangerous heart rhythm problems and is illegal in the U.S. Please talk with your pharmacist before mixing anything with it.

CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS

Acetaminophen, Chlorpheniramine, Pseudoephedrine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand: this cold and allergy combo (acetaminophen, chlorpheniramine, and pseudoephedrine) can mildly block an enzyme called CYP2D6. That same enzyme is what your body uses to clear ibogaine, the active compound in Iboga. So the worry isn't that Iboga changes your cold medicine, it's the other way around. By slowing that enzyme, this product could let ibogaine build up to higher levels in your system, much like the doubling seen when a stronger CYP2D6 blocker was studied.

That matters because Iboga already carries real risks, especially dangerous heart rhythm changes, and higher levels make that more concerning. This is largely theoretical, and ibogaine isn't legal here for casual use. Please don't combine them, and talk with your pharmacist or doctor first.
